Are Vaccines Fueling New Covid Variants?

Public-health experts are sounding the alarm about a new Omicron variant dubbed XBB that is rapidly spreading across the Northeast U.S. Some studies suggest it is as different from the original Covid strain from Wuhan as the 2003 SARS virus. Should Americans be worried? It isn’t clear that XBB is any more lethal than other variants, but its mutations enable it to evade antibodies from prior infection and vaccines as well as existing monoclonal antibody treatments. Growing evidence also suggests that repeated vaccinations may make people more susceptible to XBB and could be fueling the virus’s rapid evolution.
The subvariant of omicron, named XBB.1.5, has raised concerns about another potential wave of Covid cases following the busy holiday travel season. The CDC projected Friday that about 40% of confirmed U.S. Covid cases are caused by the XBB.1.5 strain, up from 20% a week ago. There’s no indication it causes more severe illness than any other omicron virus, Dr. Barbara Mahon, director of CDC’s Coronavirus and Other Respiratory Viruses Division, told NBC News. The XBB.1.5 is a relative of the omicron XBB variant, which is a recombinant of the omicron BA.2.10.1 and BA.2.75 subvariants. Yet, only 37.5% of that age group has received the most recent omicron booster, according to the most recent CDC data.
The Covid omicron XBB.1.5 variant is rapidly becoming dominant in the U.S. because it is highly immune evasive and appears more effective at binding to cells than related subvariants, scientists say. It has since evolved into a family of subvariants including XBB.1 and XBB.1.5. The XBB subvariants are also resistant to Evusheld, an antibody cocktail that many people with weak immune systems rely on for protection against Covid infection because they don't mount a strong response to the vaccines. The scientists described the resistance of the XBB subvariants to antibodies from vaccination and infection as "alarming." The XBB subvariants were even more effective at dodging protection from the omicron boosters than the BQ subvariants, which are also highly immune evasive, the scientists found.
Although many public health experts are expressing concern about the rising COVID cases in China, infectious disease experts have been increasingly worried about the XBB.1.5 variant. Recombinants of the BA.2 variant, XBB and XBB.1.5, together accounted for 44.1% of the total cases in the country for the week ended Dec. 31. For the week ended Dec.24, XBB.1.5 had made up 21.7% of the total cases. The XBB variant has been driving up cases in parts of Asia, including Singapore. It accounted for 3.6% of the total cases in the U.S. this week compared with 4.2% in the previous week.
Dec 23 (Reuters) - The Omicron subvariant XBB is expected to account for 18.3% of the COVID-19 cases in the United States, up from 11.2% in the previous week, the U.S. Centers for Disease Control and Prevention (CDC) said on Friday. The XBB variant has been driving up cases in parts of Asia, including Singapore, and some experts have said it is more transmissible but less severe. The variant is expected to become the dominant one in parts of the U.S. Northeast in the week ended Dec. 24, according to the CDC. Overall, the BQ.1.1 and BQ.1 remain the dominant circulating variants in the week ended Dec. 24, accounting for 63.1% of the cases, compared with 64.6% a week ago, the CDC said. The omicron subvariants that have become dominant in recent months present a serious threat to the effectiveness of the new boosters, render antibody treatments ineffective and could cause a surge of breakthrough infections, according to a new study. The BQ.1, BQ.1.1, XBB and XBB.1 omicron subvariants are the most immune evasive variants of Covid-19 to date, according to scientists affiliated with Columbia University and the University of Michigan. The immune response of people who were vaccinated and had breakthrough infections with prior omicron variants was also weaker against the subvariants. For people who received the omicron boosters, antibodies that block infection were 24 times lower against BQ.1, 41 times lower against BQ.1.1, 66 times lower against XBB and 85 times lower against XBB.1 compared to their performance against the ancestral strain that emerged in Wuhan, China in 2019. BQ.1.1 and BQ.1 represent 37% and 31% of new infections respectively, while XBB is causing 4.7% of new infections, according to CDC data.
The most immune evasive omicron subvariants yet are now causing more than 70% of new infections in the U.S., as millions of Americans prepare to travel and gather with family for the holidays later this month. Scientists, in several independent studies, have described the BQ and XBB subvariants as more adept at evading immunity from vaccination and infection than prior versions of the virus. They pose a significant threat to people with compromised immune systems because key antibody treatments are resistant to them. The Food and Drug Administration last week pulled bebtelovimab, a monoclonal antibody used to prevent people who catch Covid from developing severe illness. Many people with weak immune systems, such as organ transplant patients, cannot take Paxlovid with their other medications.

Pfizer said its omicron booster triggers a stronger immune response against a number of emerging Covid subvariants circulating the U.S. Pfizer developed its booster against omicron BA.5 at the request of the Food and Drug Administration. Among the emerging subvariants, the omicron booster elicited the strongest immune response against BA.4.6 with antibodies 11 times higher, and the weakest response against XBB.1 with antibodies about 5 times higher. Moderna on Monday said its omicron booster induces an immune response against BQ.1.1 though the company didn't provide specifics. Pfizer and Moderna's new boosters target both omicron BA.5 and the original strain of Covid that emerged in Wuhan, China in 2019.
Nov 18 (Reuters) - Pfizer Inc (PFE.N) and its German partner BioNTech SE said on Friday their Omicron-tailored shot produced higher virus-neutralizing antibodies in older adults against the emerging subvariant BQ.1.1 than its original vaccine. Antibody levels against the variant rose nearly nine-fold in older adults, aged 55 and above, who received the Omicron shot compared to a roughly 2-fold increase in participants with the original shot, according to data posted on online archive bioRxiv. The variant and related BQ.1 are gaining ground in the United States, and are expected to cause a rise in cases in the winter in Europe. The companies had recently released data that showed their Omicron-tailored shot produced a strong antibody response in older adults than the original shot after one month against the BA.4/5 subvariants. Pfizer and BioNTech said the bivalent shot also produced an immune response against newer Omicron subvariants including BA.4.6, BA.2.75.2 and XBB.1.

SYDNEY, Nov 12 (Reuters) - Australia's Home Affairs Minister on Saturday sought to reassure the public that COVID-19 protocols were adequate after a cruise ship with hundreds of infected passengers docked in Sydney. The incident has sparked comparisons with a notorious 2020 outbreak onboard the Ruby Princess cruise ship. That outbreak, also in New South Wales, led to 914 infections and 28 deaths, an inquiry found. Carnival Australia, part of global leisure company Carnival Corporation & plc, said COVID-positive passengers were isolating onboard and being cared for by medical staff, according to New South Wales Health. The agency said it was working with cruise ship staff to monitor the health of passengers and crew members.
Social media users in Singapore are copy-pasting a text post which warns that the COVID-19 Omicron XBB variant, first discovered in August, is five times more “toxic” and has a higher mortality rate than the Delta variant. However, there is no evidence to support this, according to Singapore’s Ministry of Health. Together, they now account for more than 1 in 4 new Covid-19 infections nationwide, according to CDC data. BQ.1.1 is now causing about 1 in 5 new Covid-19 infections in the Northeast, where cases and hospitalizations are rising. But that strain is causing just 3% of new Covid-19 infections in the Pacific Northwest.

It forecast that global daily deaths would average 2,748 people on Feb. 1, compared with around 1,660 currently. IHME estimates that daily infections in the United States will increase by a third to more than a million, driven by students back in schools and cold weather-related indoor gatherings. A rapid increase in hospital admissions in Germany – the highest since the COVID outbreak in 2020 - remains an area of concern, it said.

Troubling recent reports reveal the emergence of new omicron subvariants that not only evade AstraZeneca’s Evusheld, the antibody drug authorized to prevent Covid infection, but also the sole antibody drug that has retained effectiveness as treatment for Covid, Eli Lilly’s bebtelovimab. Doctors have cautioned that should Laura get Covid, this could aggravate her autoimmune disease and lead to catastrophic kidney damage. Evusheld is a long-acting antibody injection given every six months that studies have suggested provides a robust buffer to the immunosuppressed. Experts say: prepare for more COVID infections this winter. But "among the new variants, XBB has the most significant immune evasion properties," market forecasters at Morgan Stanley said Thursday in a memo. Experts say we should expect many more infections this fall and winter, including infections in vaccinated, boosted Americans. But there are some simple things you can do to prepare to battle XBB and other evasive COVID variants on the horizon. Both have a slight growth advantage over BA.5, which is the dominant version of COVID right now in the US.
