Small private practices and health-care providers are facing mounting financial pressures as crucial reimbursement systems remain down for the ninth day, following the cyberattack on Change Healthcare. Change Healthcare offers tools for payment and revenue cycle management that help facilitate transactions between providers and most major insurance companies. As of Thursday, Change Healthcare has not shared any updates about when it expects its systems to be back online. Change Healthcare on Thursday said that ransomware group Blackcat is behind the attack. He said it's not clear whether Change Healthcare will take on the responsibility of processing all the claims or if he'll need to hire additional staff to help.

But for the nation’s nursing homes, the effects have yet to fully fade, with staffing shortages and employee burnout still at crisis levels and many facilities struggling to stay afloat, according to a new report published Thursday by federal investigators. The report, by the inspector general’s office at the U.S. Department of Health and Human Services, found that the flawed infection-control procedures that contributed to the 170,000 deaths at nursing homes during the pandemic were still inadequate at many facilities. The inspector general’s report described the staffing problems as “monumental,” noting high levels of burnout, frequent employee turnover and the burdens of constantly training new employees, some of whom fail to show up for their first day of work. For nursing homes, the inability to attract and retain certified nurse aides, dietary services staff and housekeeping workers is tied to federal and state reimbursements that do not cover the full cost of care. Rachel Bryan, a social science analyst with the inspector general’s office, said the report sought to ensure that key lessons from the pandemic were not lost, especially now that the acute sense of urgency has faded.

Tony Evers signed a package of bipartisan legislation aimed at expanding access to dental care and addressing dental workforce shortages across the state into law Wednesday. U.S. Health Resources Administration data released this month found shortages of dental care in 160 areas of Wisconsin, defined as geographic areas, facilities or populations experiencing a shortage of care providers. The package Evers signed includes five bills, including one that authorizes licenses for dental therapists who practice in areas experiencing shortages in care. Dental therapists are positions between a dentist and a hygienist — they can fill cavities but can't perform more complex operations like root canals. New York Attorney General Letitia James sued Citibank for allegedly failing to protect and reimburse victims of electronic fraud, according to a Tuesday release. The suit claims that Citi does not have strong protections in place to prevent unauthorized account takeovers, misleads victims of fraud and illegally denies reimbursements. The attorney general's office said the alleged failure on Citi's part has cost New York account holders millions of dollars, and in some cases, their entire life savings. In a statement, Citi said the bank "works extremely hard" to prevent threats and assist customers who become victims of fraud. James alleged in the lawsuit that Citi must reimburse victims of fraud under the Electronic Fund Transfer Act.

The U.S. Department of Transportation on Monday said it fined Southwest Airlines $140 million for violating consumer protection laws during last year's holiday meltdown that stranded millions of customers following severe winter weather. It includes a $35 million cash payment to the government, which Southwest said will be paid over three years. Kristi Noem has requested guidance from the state’s Supreme Court about conflict-of-interest rules for lawmakers, several weeks after a South Dakota state lawmaker resigned and agreed to repay $500,000 in federal COVID-19 relief that she received for her day care business. Doing so violated a state Supreme Court advisory warning state lawmakers that it is unconstitutional for them to accept federal pandemic funding. Noem, Attorney General Marty Jackley and two Republican lawmakers have sent letters to the court seeking clarity on what else is illegal, the Sioux Falls Argus Leader reported Thursday. Putting federal money behind a program to fight climate change while using natural gas, a fossil fuel, as one of its main ingredients may not seem intuitive. To wit, the Bipartisan Infrastructure Law, where the funding for the hydrogen hubs comes from, specifically required that two of the selected hydrogen hubs be located in natural gas-rich regions. Making hydrogen from natural gas with carbon capture will require additional energy and be more expensive than producing hydrogen from natural gas where the excess carbon emissions just spew into the atmosphere. That is what the U.S. government is trying to change with its hydrogen hubs program and the production tax credit.

The nearly 4 million Americans who are federal employees will feel the effect immediately. Essential workers will remain on the job, but others will be furloughed until the shutdown is over. For many of them, a shutdown would strain their finances, as it did during the record 35-day funding lapse in 2018-2019.
