Dozens of inmates, including the disgraced financier Jeffrey Epstein, have died needlessly in federal prisons as a result of lax supervision, access to contraband and poor monitoring of at-risk inmates, according to a report released on Thursday by the Justice Department’s watchdog. The Bureau of Prisons, responsible for about 155,000 inmates, routinely subjects prisoners to conditions that put them at heightened risk of self-harm, drug overdoses, accidents and violence, the department’s inspector general found after analyzing 344 deaths from 2013 to 2021 that had not been caused by illnesses. More than half of those deaths were suicides, and many of them could have been prevented if inmates had received appropriate mental health assessments or been housed with other prisoners in accordance with departmental guidelines instead of being left alone, like Mr. Epstein, the report concluded. The report “identified several operational and managerial deficiencies” that violated standing bureau policies, said Michael E. Horowitz, the inspector general, whose investigators previously concluded that Mr. Epstein’s death at the Metropolitan Correctional Center in 2019 was the result of gross negligence and inadequate staffing.

WASHINGTON (AP) — The Biden administration struggled to properly vet and monitor the homes where they placed a surge of migrant children who arrived at the U.S.-Mexico border in 2021, according to a federal watchdog report released Thursday. In about a third of the cases reviewed by the federal watchdog, the agency did not have legible documentation for the adults on file. The federal watchdog analyzed the case files of more than 300 migrant children from early 2021, months after thousands of children had trekked to the U.S. border seeking asylum. HHS is supposed to obtain IDs for the adults – called sponsors – who take in migrant children. And, for every five cases, HHS didn’t follow up to check on the children it had placed, often for months.

The Treasury Department's corruption watchdog on Tuesday issued new proposed regulations that would extend major pieces of the anti-money laundering (AML) rules that apply to banks to some investment advisers. The proposed regulations stop short of requiring investment advisers to adopt formal customer identification programs, like banks do. MADISON, Wis. (AP) — A Republican member of the Wisconsin Elections Commission who has faced calls to resign after falsely declaring that former President Donald Trump won the state claimed Thursday that he's done more for Black people than any other white Republican. Bob Spindell, who is white, made the remark after two members of the public called for him to resign during a commission meeting. “There is no white Republican that has done more for the Black community than me, so I suggest you go back and take a look at my past record,” Spindell said. Spindell credited a “well thought out multi-faceted plan” that included recruiting strong Republican candidates and reaching out to Black voters. “It’s very, very reassuring to me that Commissioner Spindell has come out boldly and said that Joseph Biden won in 2020," Thomsen said.
