The Financial Reporting Council (FRC) said the number, range, and seriousness of the deficiencies in the audits of Carillion were exceptional, resulting in the watchdog's highest ever fine. On occasions, KPMG audit partner Peter Meehan told his team to record his review of working papers without having done a review, the FRC said. Meehan, no longer with KPMG, was fined 350,000 pounds after a discount to reflect his cooperation and admission of failures. It would have been 30 million pounds, but was discounted due to admissions and co-operation by the auditor. KPMG was fined 14.4 million pounds last year after providing false and misleading information to the FRC during spot checks on audits of Carillion and outsourcing firm Regenersis.

PITTSBURGH (AP) — Dr. Richard W. Moriarty, a retired pediatrician from Pittsburgh who helped create the bright green Mr. Yuk sticker that warns kids away from poisonous substances, has died. Moriarty passed away on Thursday, according to Jeffrey Fugh of the funeral home John A. Freyvogel Sons Inc., which is handling arrangements in Pittsburgh. Moriarty was involved in establishing and developing the Pittsburgh Poison Center, where he served as director. He told the newspaper that the response by children in focus groups to the Mr. Yuk sticker's sickly green color and upset face was instrumental — "the Mr. Yuk symbol was designed by kids for kids.” The poison awareness campaign began in 1971. Visitation is Sunday and Monday, with a private funeral on Tuesday.

LONDON, Nov 16 (Reuters) - Britain issued its first spaceport licence on Wednesday, paving the way for the country's maiden satellite launch later this year as it looks to become Europe's leading space industry player. The licence was issued to Spaceport Cornwall. The first UK launch is expected before the end of the year, the UK Space Agency's deputy chief executive, Ian Annett, said in a CAA statement. Virgin Orbit plans to use a modified Boeing 747 with a rocket attached under its wing. The CAA said Spaceport Cornwall had demonstrated it had met the appropriate safety, security and environmental requirements to become a spaceport.
