The Airbus plane had previously been used by King Charles, Queen Camilla, and other UK officials. AdvertisementA plane used by VIPs like King Charles and Queen Camilla had to make an emergency landing after three windowpanes fell out mid-flight. The lights, used to convey a sunrise, had been aimed at the windows for several hours at a time, the report said. A hole burned through one of the windows while five other windows were deformed after 2,000W lights were used, the report said. If high-intensity lights were used, it suggested keeping use to a minimum and away from windows.

Accidents are all too common, and often deadly, says Sam Mayall, a lifelong sailor and former maritime transport deck officer. “A lot of the technologies employed offshore now are the same technologies that have been there for the last 40 or 50 years,” says Mayall. Spotting people is just one part of search and rescue — getting them out of the water rapidly is also critical. Zelim’s “Swift” conveyor belt is adapted to the maritime environment to get people out of the water quickly. The maritime industry is slow to innovate, says Phillips, and the industry-standard rescue winches have been the same for decades.

A plane flying from London to Orlando, Fla., last month was forced to turn around because some windowpanes were missing or damaged, startling passengers who had reported hearing roaring sounds inside the cabin, the authorities said. The plane, an Airbus A321, had four damaged windowpanes, including two that were completely missing, as it took off from London Stansted Airport on Oct. 4, according to a report published last week by the British Air Accidents Investigation Branch. There were no injuries aboard the flight, which had 11 crew members and nine passengers who sat in the middle of the aircraft and who were all employees of a “tour company or the aircraft’s operating company,” the authorities said.

CNN —An Airbus A321 aircraft took off from London Stansted Airport last month with four damaged window panes, including two that were missing, according to UK air accident investigators. The plane landed back at Stansted Airport safely shortly afterward. “Having inspected the window, it was agreed the aircraft should return to Stansted,” the report continues. The flight crew initiated a descent and the aircraft arrived back at Stansted Airport a short while later. After passengers had disembarked and the plane was parked and shut down, the crew inspected the plane from the outside, and found that two cabin window panes were missing and a third was dislodged.

By Kylie MacLellanLONDON (Reuters) - Unsuccessful attempts to rescue migrants from a sinking boat in the Channel in November 2021 were hampered by poor visibility and confusion over locating the right vessel, Britain's Marine Accident Investigation Branch (MAIB) said on Thursday. "The UK search and rescue response ... was hampered due to poor visibility and by the lack of a dedicated aircraft conducting aerial surveillance," it said in a report published on Thursday. The Channel between France and Britain is one of the world's busiest shipping lanes and currents are strong, making the crossing on small boats dangerous. On the day of the deaths, 1,227 people attempted to cross the Channel, the British government said. More than 25,000 people have arrived in small boats so far in 2023.

The convening of a Marine Board of Investigation is the highest level of investigation conducted by the U.S. Coast Guard, Neubauer said. Investigators are working closely with other national and international investigative authorities, including the U.S. National Transportation Safety Board, the Transportation Safety Board of Canada, the French marine casualties investigation board and the United Kingdom Marine Accident Investigation Branch, Neubauer added. The Coast Guard board can make recommendations to prosecutors to pursue civil or criminal sanctions as necessary. The Coast Guard led the initial search and rescue mission, a massive international effort that likely cost millions of dollars. "The Coast Guard doesn't charge for search and rescue nor do we associate a cost with human life," said Rear Adm. John Mauger, of the Coast Guard First District.

A flying instructor died during a private flight last year, according to a new safety report. The pilot thought his instructor was "just pretending to take a nap," before realizing he was dead. The pilot knew the instructor well and thought he was simply "pretending to take a nap," not realizing what had occurred, the report said. The Civil Aviation Authority found three previous instances of a pilot suffering a heart attack inflight since 2005, according to the accidents report.
The UK government has promised to overhaul the youth gender care system, after it was deemed inadequate by England’s regulator of health and social care. They described a deeply flawed system that is now hobbled by a toxic political climate around gender care. The letter said a decision would be made at some point from early 2022 on whether the child “is likely to meet the access criteria” for gender care. The family has received no NHS gender care or mental health support since the referral, she said. These recommended supporting “identity exploration” and mental health treatment as the first steps to ensure that any psychological issues are addressed.
New Delhi CNN Business —Aftershocks from the massive earthquake in the trillion-dollar crypto industry last week continued to be felt on Monday. The episode has not just destroyed confidence in the crypto industry, but it will also embolden global regulators to tighten the screws. There is a “lot of risk,” said Changpeng Zhao, who runs the crypto exchange Binance. On Saturday, FTX said it was looking into whether crypto assets were stolen and has since moved all its digital assets offline. Crypto risk management firm Elliptic said although the theft was unconfirmed, $473 million in crypto assets were apparently stolen from FTX.

New York CNN —Authorities in The Bahamas are investigating potential criminal misconduct surrounding the stunning implosion of crypto exchange FTX, authorities said on Sunday. In a statement, the Royal Bahamas Police Force confirmed the investigation into FTX, which is headquartered in The Bahamas. It’s not clear which particular aspect of the swift collapse of FTX authorities are investigating. Previously one of the most trusted brands in crypto, FTX filed for bankruptcy on Friday and announced the resignation of Sam Bankman-Fried, its 30-year-old CEO who has lost a fortune in the past week. Neither FTX nor a lawyer representing Bankman-Fried responded to requests for comment on Sunday in response to the investigation from authorities in The Bahamas.
Bahamian police questioned FTX founder Sam Bankman-Fried on Saturday, Reuters reported. The police say they are investigating if "criminal misconduct occurred" tied to the downfall of FTX. Bankman-Fried was rumored to have fled to Argentina earlier this weekend. The Royal Bahamas Police questioned Bankman-Fried to discover if any "criminal misconduct occurred," Reuters reported on Sunday. The Financial Times later reported that FTX held only $900 million in sellable assets against $9 billion in liabilities.
