A 23-year-old construction worker died in March when an overloaded crane collapsed. Its 15 violations included failing to create or employ an accident prevention program. AdvertisementAdvertisementOSHA cited S&D Erectors with a total of 15 violations, including operating a mobile crane in excess of its rated capacity. The federal agency also said that S&D Erectors did not create and employ an accident prevention program, or perform frequent and regular inspections of the site.
