Maui fire commanders discussed the forecast, but “no evidence of pre-event preparedness plans by the MFD were produced,” the report said. Maui County and the state use private contractors to help fight fires with water tankers and heavy equipment. BOSTON (AP) — Investigators have identified two individuals allegedly involved in a shooting that claimed the life of a baby delivered after its mother was one of several people hit by gunfire during a fight Wednesday in downtown Holyoke, Massachusetts. Alejandro Ramos, 22, of Holyoke, is scheduled to be arraigned in Holyoke District Court on Thursday for his alleged involvement in the shooting, investigators said. An official at the Hampden District Attorney’s Office said it was unclear if Ramos had been appointed an attorney yet. Police responded to the shooting Wednesday at 12:38 p.m. and said it appeared three male suspects were involved in an altercation before gunshots were fired. The Massachusetts State Police Detective Unit assigned to the Hampden District Attorney’s Office and the Holyoke Police Department are investigating.

The chief investigator for the fire marshal’s office, Adam Matzner, said Tuesday that investigators haven’t found any sign of a criminal act connected with the explosion, so the incident appears accidental. A spokesman for the Federal Railroad Administration said Tuesday that he couldn’t provide any additional details about the agency’s investigation. WASHINGTON (AP) — Investigators said Thursday that a failed radio transmission and a distracted air traffic controller led to a close call between a Southwest Airlines jet and a smaller plane waiting to take off from San Diego in June. The National Transportation Safety Board said a controller told the crew of SkyWest plane to exit the runway, then directed the Southwest pilots to cut short their landing approach and instead circle around the airport. After that, according to the NTSB, audio recorded by LiveATC captured a Southwest pilot saying, “Ah, is that an airplane on the runway?" The SkyWest crew said they heard that while they were moving to leave the runway. Federal officials are still looking into another close call in San Diego in August in August between a Southwest plane and a business jet.
