A helicopter that crashed into a Houston radio tower Sunday, killing all four people aboard and starting a fire below, was on an air tour flight when the accident occurred, the National Transportation Safety Board said Monday. The Robinson R44 helicopter struck the tower and crashed at around 7:51 p.m. Sunday, said the NTSB, which is investigating. The helicopter took off from Ellington Airport, which is in Houston, the NTSB said. Multiple people died as a helicopter crashed into a radio tower in Houston on Sunday. Pamela Helmstadter went to get help, but “got disoriented in the woods and could not get home,” the Maine Game Wardens said in a news release. After spending four nights in the woods, she was located on Thursday — alive, but hypothermic — by the Maine Game Wardens and warden K9s, over a mile from her home. In the days the couple were lost in the woods, the temperature dipped to as low as 26 degrees Fahrenheit at night, according to Accuweather. Window Rock, Arizona AP —John Kinsel Sr., one of the last remaining Navajo Code Talkers who transmitted messages during World War II based on the tribe’s native language, has died. With Kinsel’s death, only two Navajo Code Talkers are still alive: Former Navajo Chairman Peter MacDonald and Thomas H. Begay. Hundreds of Navajos were recruited by the Marines to serve as Code Talkers during the war, transmitting messages based on their then-unwritten native language. The Code Talkers sent thousands of messages without error on Japanese troop movements, battlefield tactics and other communications crucial to the war’s ultimate outcome. President Ronald Reagan established Navajo Code Talkers Day in 1982 and the August 14 holiday honors all the tribes associated with the war effort.

The House task force on the attempted assassinations of former President Donald J. Trump released its first preliminary report Monday, chastising the Secret Service for poor planning and a communications breakdown that resulted in a fatal shooting at Mr. Trump’s rally in Butler, Pa.“Put simply, the evidence obtained by the task force to date shows the tragic and shocking events of July 13 were preventable and should not have happened,” the report said. The 53-page bipartisan report is based on 23 interviews with local law enforcement officials who assisted with security at the rally, thousands of pages of documents from local, state and federal authorities, and testimony from the task force’s public hearing on Sept. 26. Its findings largely hew to the conclusions of three other recent investigations. A Senate inquiry found that no one was in charge of planning and security decisions for the July 13 campaign rally where Mr. Trump was shot; an independent panel reviewing the failures that led to the attempted assassination called on the Secret Service to replace its leadership with people from the private sector and focus almost exclusively on its protective mission; and the Secret Service’s own internal investigation found that complacency had set in among some of the agents charged with securing the rally site.

Eutelsat, the world's third-biggest satellite operator by revenue, launched 20 satellites for its communications network on Sunday, using Elon Musk's SpaceX in its first move since the merger of two European companies last year. A SpaceX Falcon 9 rocket took off, with Eutelsat satellites from California's Vandenberg Space Force Base at 0513 GMT. "This is the first OneWeb launch of the satellites since the merger," CEO Eva Berneke told Reuters in an interview. The Paris-based group formed by the merger in September last year of France's Eutelsat and Britain's OneWeb has a constellation of over 600 low earth orbit satellites that cater to broadcasters, telecom companies and radio stations. It sits there until India gets open, the day it gets open, we'll start building," Berneke said.

CNN —Multiple people were killed, including a child, when a helicopter slammed into a communications tower behind homes in Houston’s Second Ward on Sunday night, causing a fiery explosion and toppling the structure, Mayor John Whitmire said. Officials did not clarify how many were killed, but there were four people onboard the privately-owned Robinson R44 helicopter, according to Houston Police Chief Noe Diaz. The aircraft appears to have departed the Ellington Airport, which is about 17 miles south of the crash site, but its destination is unclear, the mayor said. Photos shared by the fire department show the mangled tower draped across a dirt clearing behind a line of homes. Some homes near the crash site lost power, Whitmire said.
