MEXICO CITY, Nov 22 (Reuters) - The Mexican National Guard on Wednesday captured Nestor Isidro Perez Salas, or "El Nini," who is accused of heading security for the faction of the Sinaloa Cartel headed by the sons of founder Joaquin "El Chapo" Guzman. Seen by U.S. anti-narcotics agents as one of the most ruthless Mexican drug figures, Perez Salas was detained in Culiacan, the Sinaloa Cartel's heartlands, and was taken to Mexico City, according to government detention records. "We're working side-by-side to combat arms trafficking, to tackle organized crime and to address the opioid epidemic, including fentanyl," Biden said. The U.S. and Mexico also agreed with China last week to stem the flow of fentanyl precursor chemicals from the Asian country. In January, Mexican authorities captured Ovidio Guzman in Sinaloa and extradited him to the United States in September.

Costa Rica makes first-ever fentanyl gang arrests

Four suspects were taken into custody, two Costa Ricans and two Colombians, along with 1,100 fentanyl pills in an operation backed by the U.S. Drug Enforcement Administration that included raids in three towns in central Costa Rica, Security Minister Mario Zamora said at a press conference. The arrests "raises alarms because it confirms the presence of fentanyl" in Costa Rica, said Zamora. As of last month, Costa Rican authorities had been carrying out 10 fentanyl investigations since last year, according to government data. Violent crime in Costa Rica has surged this year with homicides at a record, and the growth blamed on gang turf battles over the country's role as a major transit point for the illicit trade. The fentanyl arrests come as around 70% of Costa Ricans disapprove of President Rodrigo Chaves' crime fighting record, according to a recent poll.

A pivotal new study suggests that the weight loss drug Wegovy cut the risk of heart attack, stroke or death from cardiovascular issues by 20 percent among overweight or obese people with heart disease — a striking benefit that could change the standard of care for these patients. “We’ve just identified a new best practice,” said Dr. Clyde Yancy, chief of the division of cardiology at Northwestern Medicine, who was not involved with the study. Given that patients had not started losing weight when the cardiovascular benefits first appeared suggests the heart protection was not purely the result of weight loss, Novo said. Though the trial was not conducted to test weight loss, participants lost an average of nearly 10% of their total body weight. Novo said patients in the heart study were not required to track diet and exercise as they are in obesity trials.

The FDA approved tirzepatide for weight loss under the brand name Zepbound. It has been available as Mounjaro for type 2 diabetes since 2022 and had increasingly been used "off-label" for weight loss while the obesity approval was pending. Zepbound will be available in the U.S. by the end of the year at a list price of $1,059.87 a month, according to Lilly. That compares with a list of $1,349 per-package for Novo Nordisk's (NOVOb.CO) wildly popular weight-loss drug Wegovy. After Wednesday's FDA approval, Lilly can now promote the drug for weight loss.

SAN JUAN, Puerto Rico (AP) — One of Puerto Rico’s biggest distributors of pharmaceutical drugs was ordered to pay $12 million after being accused of not reporting hundreds of suspicious orders for controlled substances, the U.S. Justice Department said Monday. Drogueria Betances, LLC is accused of not reporting at least 655 orders for fentanyl and at least 113 orders for oxycodone from 2016 through June 2019. The orders were considered suspicious given in part their frequency and size, the department said. In addition, the company was ordered to improve its compliance program and report suspicious orders to the DEA, among other things. He added that Drogueria Betances never was accused of diverting controlled products to unauthorized people or entities.

Eli Lilly on Thursday said supply of its blockbuster diabetes drug Mounjaro has improved in the U.S. after months of widespread shortages. During an earnings call Thursday, Eli Lilly executives said the company experienced tight supply for Mounjaro throughout most of the third quarter. But U.S. product shipments of Mounjaro have recently increased, and inventory levels at U.S. drug wholesalers have improved, according to Eli Lilly CFO Anat Ashkenazi. Eli Lilly has also invested more than $3 billion to build two new manufacturing sites in its home state of Indiana. Lilly has said it expects U.S. approval for Mounjaro for weight loss by the end of the year. The source told Reuters that CordenPharma's factory in Colorado is under contract to make tirzepatide, the active ingredient in the weekly injection. Analysts estimate the obesity market could be worth as much as $100 billion in the next decade.
