A top exchange-traded fund provider is betting on the long-term popularity of GLP-1 weight loss drugs. Roundhill Investments' GLP-1 & Weight Loss ETF (OZEM), which began trading last week, pairs leaders Eli Lilly and Novo Nordisk with players developing new treatments for weight loss and diabetes. Eli Lilly and Novo Nordisk each hold a roughly 20% weighting in the ETF, per Roundhill's website as of Friday. Mazza waived concerns that investors have missed out on the rally, noting the weight loss drug industry is still in its "early days." After last Tuesday's launch, shares of Roundhill's GLP-1 & Weight Loss ETF ended the week down by almost 2%.

Weight-loss drugs like Ozempic, Wegovy, and Mounjaro are all the rage, and investors are noticing. Since launching in November, the Tema Obesity and Cardiometabolic ETF (HRTS) is up 24%, crushing the S&P 500's 14.8% returns over the same period. The private company plans to start enrollments for the trial of the drug, survodutide, which it co-invented with Danish biotech company Zealand Pharma (ZELA.CO), before the end of the year. Boehringer and Zealand are among global drugmakers racing to grab a share of the potential $100 billion market for obesity treatments within a decade. Survodutide works by mimicing a gut hormone called glucagon-like peptide-1 (GLP-1), which suppresses appetite, as well as imitating another gut hormone called glucagon that helps break down fat.

Wegovy was approved as an obesity treatment in June 2021 , but Ozempic, a Type 2 diabetes treatment, has been used off-label for weight loss. Mounjaro uses GLP-1 and gastric inhibitory polypeptide, or GIP, to help patients lose weight. Eli Lilly's oral drug, orforglipron, has an even better profile . They also could be helpful in assisting patients in maintaining weight loss after they've used the injectable versions, he said. Some have highlighted that oral drugs also may be more manageable for patients who can't handle the idea of giving themselves a once-a-week injection.

More than 40% of Americans are obese, according to the U.S. Centers for Disease Control and Prevention, and demand is strong for treatments to reduce body weight as well as maintain weight loss. The company said it would continue to develop its twice-daily weight loss drug candidate. Structure Therapeutics (GPCR.O) is developing oral obesity drugs that it says are simpler to manufacture than the current injectables. "We define weight loss quality as the percentage of weight loss attributed to fat loss," said Versanis Chief Scientific Officer Lloyd Klickstein. "With diets, bariatric surgery, incretin drugs or other weight loss drugs, two-thirds to three-quarters of the weight loss is fat, but one-quarter to one-third is lean."

In a statement on Wednesday, the partners said that the Phase II dose-finding trial met its primary endpoint of weight loss, the Danish company added. Like the other drugs, BI 456906 works by reducing the effect of a gut hormone called glucagon-like peptide-1 (GLP-1) that is involved in regulating appetite and food intake. But BI 456906 also blocks another gut hormone called glucagon. Mounjaro also has a dual mechanism of action, but it blocks yet another gut hormone in addition to GLP-1. According to drug trial database clinicaltrials.gov, participants in the Zealand/Boehringer trial were put into five groups by chance, with four getting different doses of the experimental weekly shot and the fifth getting a placebo.
The company has two oral GLP-1 drugs in mid-stage trials, and aims to choose one for a late-stage trial this year. He forecasts GLP-1s or similar drugs topping $100 billion in annual sales early in the 2030s, with Lilly's product accounting for more than $50 billion in sales. Smaller biotechs are also vying for a piece of the obesity market and hope large pharmaceutical companies will pay up for partnerships.
