The pilots of the London-bound American Airlines Boeing 777 took a wrong turn on a taxiway alongside two perpendicular runways. Instead, they crossed 4L just as a Delta Boeing 737 began its takeoff roll down the same runway. A controller warned the American crew about a “possible pilot deviation," and gave them a phone number to call, which the captain did. The cockpit voice recording from inside the American plane was taped over during the six-hour flight to London and lost forever. Investigators said they tried several times to interview the American pilots, but the pilots refused on advice of their union, which objected to the NTSB recording the interviews.

A near-total breakdown in policing protocols hindered the response to the 2022 school shooting in Uvalde, Texas, that left 21 people dead — and the refusal to rapidly confront the killer needlessly cost lives, the Justice Department concluded on Thursday after a nearly two-year investigation. The department blamed “cascading failures of leadership, decision-making, tactics, policy and training” for the delayed and passive law enforcement response that allowed an 18-year-old gunman with a semiautomatic rifle to remain inside a pair of connected fourth grade classrooms at Robb Elementary School for 77 minutes before he was confronted and killed. The “most significant failure,” investigators concluded, was the decision by local police officials to classify the incident as a barricaded standoff rather than an “active-shooter” scenario, which would have demanded instant and aggressive action. Almost all of the officials in charge that day have already been fired or have retired. Attorney General Merrick B. Garland, speaking to reporters in Uvalde, said that the officers who converged on the school within minutes of the attack intended to storm the classrooms, but were told to stand down.

George Santos has said he expects to be expelled from Congress following a scathing report by the House Ethics Committee that found substantial evidence of lawbreaking by the New York Republican. Madison Mogen, 21, Kaylee Goncalves, 21, Xana Kernodle, 20, and her boyfriend, Ethan Chapin, 20, were found fatally stabbed at an off-campus home in Moscow, Idaho, on Nov. 13, and the killings remain a mystery. On Wednesday, the Moscow Police Department said this was a “miscommunication.”“Detectives do not currently know if the residence or any occupants were specifically targeted but continue to investigate,” police said. Flowers are left at a makeshift memorial honoring four slain University of Idaho students in Moscow, Idaho. Such unclear answers may have given whoever fatally stabbed the students more time to flee, law enforcement experts say.
The man accused of brutally attacking the husband of House Speaker Nancy Pelosi, D-Calif., appeared in state court Tuesday to face attempted murder and other charges. California prosecutors also charged David DePape, 42, with assault with a deadly weapon, elder abuse, residential burglary, false imprisonment and threatening a public official in connection with Friday's attack on Paul Pelosi. DePape specifically targeted the Pelosis' home to confront the House speaker, not realizing she was in Washington, D.C., at the time, Jenkins alleged. Based on DePape's alleged statements and comments made in the house during the encounter with Paul Pelosi, the attack appeared to be "politically motivated," Jenkins said. Investigators allege DePape attacked Paul Pelosi with a hammer after police arrived at the home early Friday morning.
